Terms of Service & Refund Policy

1. Independent Third-Party Retrieval Service
USMedicalRecords.com is an independent, private document retrieval and litigation support service. We are not affiliated with, endorsed by, or an official contractor for any hospital system, clinic, pharmacy, laboratory, government agency, or Release-of-Information (ROI) vendor (including Datavant, Ciox Health, MRO Corp, Verisma, or Sharecare). Patients may always request their own medical records directly from their healthcare provider or official patient portal without using our paid coordination service.
2. Coordination Fees vs. Statutory Facility Copying Fees
Our service fee ($89 Standard or $139 Priority Rush per facility, plus $35 if a Certified Custodian of Records Affidavit is selected) covers packet preparation, 3-department routing (Clinical HIM, Patient Financial Services Billing, and Radiology PACS), statutory fee auditing, and persistent follow-up until delivery. Under 45 CFR § 164.524(c)(4) (for direct-to-patient requests) and state statutory fee schedules (for third-party attorney requests post-Ciox Health, LLC v. Azar), healthcare facilities or their ROI vendors may assess a separate statutory copying fee. We audit every facility invoice to enforce federal and state statutory caps before any pass-through fee is approved.
3. Refund & Cancellation Policy
If you cancel your order prior to packet dispatch to the target healthcare facility, you will receive a 100% refund of your coordination fee. Once our compliance desk has prepared and dispatched the formal retrieval packet to the facility's HIM, Billing, or Radiology department (or issued a Certificate of No Records when a facility confirms no treatment occurred within the requested dates), the coordination fee is earned in full.
4. No Legal or Medical Advice
USMedicalRecords.com is not a law firm or healthcare provider. Statutory summaries, state copying fee tables, and subpoena timelines provided on this website are for informational and compliance auditing purposes only and do not constitute formal legal representation.
